醫(yī)學(xué)論文范文:清熱降濁方對(duì)2型糖尿病合并代謝綜合征患者胰島β細(xì)胞功能的影響
【摘要】 目的 探討清熱降濁方對(duì)2型糖尿病合并代謝綜合征患者胰島β細(xì)胞功能的影響。方法 采用隨機(jī)對(duì)照的臨床試驗(yàn),觀察清熱降濁方(治療組)與二甲雙胍(對(duì)照組)對(duì)208例2型糖尿病合并代謝綜合征患者糖化血紅蛋白(HbA1c)、空腹血糖(FBG)、餐后2 h血糖(P2BG)、總膽固醇(TC)、三酰甘油(TG)、高密度脂蛋白膽固醇(HDL-C)、低密度脂蛋白膽固醇(LDL-C)、體重、體重指數(shù)(BMI)、腰圍及胰島素抵抗指數(shù)(HOMA2-IR)、胰島素敏感指數(shù)(HOMA2-S)、胰島β細(xì)胞功能指數(shù)(HOMA2-B)的影響。結(jié)果 2組患者治療后HbA1c、FBG和P2BG水平均較治療前明顯下降(P<0.001),組間差異無(wú)統(tǒng)計(jì)學(xué)意義;TC、TG、LDL-C水平較治療前均有所下降,但對(duì)HDL-C的作用不明顯。治療組體重與BMI下降明顯,優(yōu)于對(duì)照組(P<0.05)。2組HOMA2-IR、HOMA2-S、HOMA2-B的改善不明顯。結(jié)論 清熱降濁方對(duì)2型糖尿病合并代謝綜合征患者各項(xiàng)指標(biāo)的影響與二甲雙胍相當(dāng),但對(duì)胰島β細(xì)胞功能的改善不明顯。

Abstract：Objective To evaluate the effect of Qingre-Jiangzhuo prescription on islet β cell function of type 2 diabetes mellitus (T2DM) combined metabolic syndrome patients. Methods Two hundred and eight T2DM patients were divided into 2 groups in a randomized, controlled clinical trail. One group was treated by metformin tablet and the other by Qingre-Jiangzhuo prescription, both for 12 weeks. HbAlc, FBG, P2BG, TC, TG, LDL-C, BW, BMI, WC and HOMA2-IR, HOMA2-S, HOMA2-B level before and after treatment were recorded. Results After treatment, FBG, P2BG and HbAlc of the treated group were significantly lower than that before treatment (P<0.001), the difference between the two groups was not significant. TC, TG and LDL-C of both groups were significantly lower than that before treatment, but not in HDL-C. Weight and BMI were significantly decreased in the treated group compared with the control group (P<0.05). But the difference in HOMA2-IR, HOMA2-S, HOMA2-B were not significant. Conclusion Qingre-Jiangzhuo prescription was same as metformin tablet in T2DM combined metabolic syndrome patients, while not in β cell function.

前期研究顯示,清熱降濁方與二甲雙胍有相似的降糖、調(diào)脂、減肥等作用,且安全性良好[1-2]。為進(jìn)一步探討清熱降濁方對(duì)2型糖尿病合并代謝綜合征患者胰島β細(xì)胞功能的影響,筆者擬通過(guò)隨機(jī)對(duì)照的臨床試驗(yàn),對(duì)患者的胰島素抵抗指數(shù)(HOMA2-IR)、胰島素敏感指數(shù)(HOMA2-S)、胰島β細(xì)胞功能指數(shù)(HOMA2-B)進(jìn)行觀察。
