醫(yī)學(xué)免費論文:2型糖尿病血漿內(nèi)脂素與糖脂代謝關(guān)系
【摘要】 目的:研究2型糖尿病(T2DM)患者血漿內(nèi)脂素水平與糖脂代謝之間的關(guān)系。方法:選取48例初診的T2DM患者(超重組27例，正常組21例)及32例健康對照，分別測量各組體重指數(shù)(BMI)、腰圍、腰臀比、血壓，并行空腹血漿內(nèi)脂素、血脂、血糖、葡萄糖耐量(OGTT)及胰島素釋放試驗(IRT)檢測，計算胰島素抵抗指數(shù)(HOMA-IR)及胰島素分泌指數(shù)(HOMA-B)，并進(jìn)行統(tǒng)計學(xué)分析比較。結(jié)果:T2DM患者血漿內(nèi)脂素明顯高于對照組(P<0.01)，而在超重組與正常組間則無顯著差異。血漿內(nèi)脂素濃度與空腹血糖FPG、餐后2 h血糖、胰島素抵抗指數(shù)呈正相關(guān)(P均<0.01)，與胰島素分泌指數(shù)(HOMA-B)呈負(fù)相關(guān)(P<0.01)。多元線性逐步回歸分析表明，餐后2 h血糖是影響血漿內(nèi)脂素的獨立相關(guān)因素。結(jié)論:初診T2DM患者血漿內(nèi)脂素顯著升高，可能是機(jī)體對體內(nèi)血糖增高、胰島功能受損所發(fā)生的一種代償反應(yīng)。

Study on the Relationship between Plasma Visfatin and Glucose Lipid Metabolism in Type 2 Diabetes MellitusWAN Chun-hui，CHEN Zhan-feng,ZHOU Meng Cadres Ward，2Divison of Medical Affairs，Taihe Hospital，Yunyang Medical College,Shiyan，Hubei 442000,China)Abstract: Objective To investigate the relationship between plasma visfatin level and glucose lipid metabolism in type 2 diabetes mellitus.Methods Forty-eight newly diagonosed patients with type 2 diabetes mellitus(T2DM) including 27 cases of overweight and 21 cases of normal weight and 32 healthy controls were enrolled.The indexes of BMI,WC,WRH,blood pressure were determined;the fasting plasma visfatin,lipid,glucose,OGTT and IRT were detected;insulin resistance index(HOMA-IR) and insulin secretion index(HOMA-B) were caculated and statistical analysis was conducted.Results The level of plasma visfatin in T2DM patients was significantly higher than that of control group(P<0.01),but there was no significant difference between overweight group and normal weight group.The level of plasma visfatin was positively correlated with fasting plasma glucose(FPG),postprandial 2 h blood glucose(2h PG),HOMA-IR(allP<0.01),and negatively correlated with HOMA-B(P<0.01).Multiple linear stepwise regression analysis showed that 2h PG was an independent related factor of plasma visfatin.Conclusion The increased plasma visfatin level in newly diagnosed T2DM patients may be a compensatory response against the increased blood glucose and impaired insulin function醫(yī).學(xué).全.在.線m.zxtf.net.cn.

內(nèi)脂素(visfatin)是一種主要由內(nèi)臟脂肪組織分泌的具有胰島素樣效應(yīng)的脂肪細(xì)胞因子,它通過作用于胰島素受體(結(jié)合位點不同)減少肝糖的輸出，刺激脂肪細(xì)胞及肌細(xì)胞對葡萄糖的利用而起到降糖作用。它的發(fā)現(xiàn)引發(fā)了人們對機(jī)體糖脂代謝平衡、脂肪細(xì)胞增殖分化的重新認(rèn)識，它可能是糖尿病與肥胖的共同發(fā)病機(jī)制中的一個重要環(huán)節(jié)[1]。本研究目的在于探討了血漿visfatin在T2DM人群中的變化以及與血糖及胰島功能的意義。
